Open Tryouts For 2004 Cougar Soccer Squad

Open Tryouts for the University of Houston Women?s Soccer Team will be held on Friday, August 20, 2004, at 10:30am at the Soccer/Track complex located directly behind the Athletics Alumni Center. Tryouts are open to any current UH student or incoming freshman enrolled at the University of Houston. Prior to any physical participation in the tryout process you will need the following information completed and approved by UH staff no later than August 13.
1. Acceptance of Risk Liability Waiver Form (must be signed by parent if under 18 years of age)
2. A copy of a Sports Physical dated within 12 months of tryout date
3. A copy of your insurance card
4. NCAA compliance packet
5. Registered with NCAA Clearinghouse
6. Enrolled full time (12 credit hours) at University of Houston
